Examining closer you find it almost imposible to reach the top of the boxes. Once the thug walking has been taken care of, climb the ladder. To the left of you, there is a big box blocking you from a ground-level box that would be a good advantage in retrieving the item. To get to it, you must jump off the box you are standing on and curve around the big box while you are gliding. This may prove to be quite difficult, and can take a while to master. Once you have succeeded, jump over to the item. You will find that it is an envelope with the letter "B" on it. When you finish the game the infinite health cheat will be unlocked.
